Records Requests

The instructions below explain how to request the healthcare records from this licensee.

Records Requests must be made in person or with a verification of Identification (Texas Photo ID required). Medical Records are only released through the Office Manager (Artemio Ortiz: 956-536-8363 or email aortiz@anewtodaycounseling.com).

Please note that, as per the Texas Administrative Code, Client Records are only kept by this Counseling Agency for seven (7) years from the date of last contact, or five (5) years beyond the age of eighteen (18) for minor clients, whichever is greater. Fees for Medical Records will apply.
